Learning you, or someone you love, have been diagnosed with a life-limiting illness is unimaginable. Knowing what to do next can be overwhelming.

Palliative Care Connect is a state wide service in South Australia that provides information and connections to palliative care and bereavement for individuals with life-limiting illnesses, as well as their families, friends, and carers.

Funded by the Australian Government Department of Health and Aged Care, Palliative Care Connect offers navigation services in person, and via a free phone service and website designed to guide and support individuals in accessing the care they need.

Palliative Care Navigators provide information, connections to appropriate services, and empower individuals to make choices that align with their cultural and personal preferences. They can help you to organise community supports or referrals to other services, planning and troubleshooting for better end of life care, check of caregivers’ needs, and deep listening of a situation and related concerns.

Sonder, a not-for-profit organisation, is delivering the Palliative Care Navigation Service throughout areas in regional South Australia, including Yorke, Lower North, and Barossa.

Support is also available in-person at Sonder’s offices located in Kadina, Clare, Nuriootpa, or Port Pirie.
