How Palliative Care Connect can help

Palliative Care Connect services are free and available statewide – no referral needed. Whether you’re living with a life-limiting illness, caring for someone, or grieving a loss, support is just a phone call away.

Navigators are experienced professionals who help people find their way through what can feel like a maze of services. They’ll explain your options, connect you with services, and help you make choices that reflect yours or your loved ones’ needs, culture, and preferences.

They can assist with:

  • Advance care planning
  • Arranging respite services
  • Sourcing equipment
  • Communicating with others involved in the person’s care​
  • Navigating aged care pathways, including the Support at Home End-of-Life Pathway

With this support, families spend less time worrying about what to do and more time focusing on what’s important to them.

Support at Home End-of-Life Pathway and Palliative Care Connect

The Support at Home End-of-Life Pathway is designed to offer coordinated, flexible support that prioritises dignity and comfort, helping people remain in their familiar home environment at the end of life.

This new pathway provides up to $25,000 in additional aged care supports over 12 weeks. It is available once per participant.

Palliative Care Connect Navigators can help families:

  • Confirm eligibility
  • Explain available supports
  • Connect families with providers quickly

Eligibility

To access the End-of-Life Pathway, the following criteria apply:

  • A prognosis of three months or less, determined by a doctor or nurse practitioner.
  • Spending more than half of the day in bed and need moderate to maximum assistance with self-care.
