An ACD is a legal document in South Australia and allows you to record what matters most to you, including the types of health care you would or would not want, who should speak for you, and your preferences for future care.

Free ACD kits are available from one of many collection points, including Service SA locations and selected libraries. Visit advancecaredirectives.sa.gov.au to create an ACD online, access helpful resources or find a collection point near you.
